Band 5 Nurse Assessor

Reporting to: Home Care Manager 

This is an excellent opportunity for a Qualified band 5 Nurse, to utilise their clinical knowledge clinical skills to further their career.

Responsibilities:

• You will join an established home care team to lead and undertake assessments on a daily basis. The role allows you to utilise your clinical and evidenced based knowledge, interpersonal techniques and skills to determine individual needs for potential and current clients
• Carrying out comprehensive assessments reviews and analysis will enable you to complete a robust, person-centred care plan and risk assessments, containing a detailed history of clients condition(s) including information gathered on the functional impact of their health condition or impairment on their daily living and mobility, their current medication and treatment.
• The main objective is to ensure a quality-of-life driven care plan, promoting independence and autonomy where possible
• You will promote best practice and provide professional advice to the home care team, promoting good relationships and liaising with colleagues from other disciplines both internally and externally.

Requirements:

• Qualified Nurse,
• 1+ years post qualification experience
• Valid registration on NMC
• Excellent report writing skills with attention to detail

Experience:

• Nursing: 1 year (required)
